Voluntary Life and AD&D Insurance
Life and Accidental Death and Dismemberment (AD&D) insurance provides financial protection for loved ones should the insured individual pass away! Operation Smile pays for employees’ Basic Life/AD&D coverage and makes Voluntary Life/AD&D available, at group rates, so employees can purchase coverage for themselves and their dependents. The chart below provides a summary of the coverage.
Mutual of Omaha will grandfather existing Voluntary Life and AD&D elections for those that enroll!
Designate Your Beneficiary
When you enroll online for benefits, look for the prompts to designate a beneficiary for your Operation Smile Company-provided life insurance coverage and/or your optional life insurance plans. If a beneficiary is not designated, benefits will be paid according to the carrier policy. It is not necessary to add a beneficiary for your spousal life or child life insurance coverage since you are automatically the beneficiary for any spouse or dependent coverage you elect
Evidence of Insurability
You may be asked to provide Evidence of Insurability (EOI) or proof of good health, if:
- You do not enroll for coverage when first available and choose to enroll later.
- You want to increase your coverage after your initial enrollment.
